This  form is to be filled out by those who are interested in supporting Paws and Stripes through a third-party event or host the Paws Team at your next fundraiser.

Paws and Stripes operating hours are, Monday - Thursday 8:00 AM - 6:00 PM. Though, we often have events and third-party fundraisers on the weekend. Once your application has been reviewed, we will contact you for your availability. ​
